The Brokerage Options Program provides short term case management for people with disability. The service is available to people with intellectual, physical, sensory, multiple disabilities as well as acquired brain injury who are adults 18 to 65 and living in the Hawkesbury and Blue Mountains LGA's. There are also brokerage funds available for case management.

FEE Cost of materials plus $15.00 per hour
Provides home modification with some maintenance to people of any age with a moderate to severe disability including frail aged and their carers. Modifications cannot be undertaken until a formal assessment has been conducted by an occupational Therapist - this can be arranged prior to contacting Fusion by making contact with local ACAT/Community Health Services. Total cost of modification must not exceed $5,000. It should be noted that due to limited resources, maintenance services receive a low priority and have a long waiting time. HACC funded.

Provides a range of services including: personal care assistance, housework, shopping, respite care, aimed at enabling frail aged and disabled people to remain at home and living independently for as long as possible. Fees for services are negotiable and are dependent on the service(s) being provided. This service covers an area from Penrith to Parramatta to Windsor and is available to frail aged people or people with disabilities who are incontinent. Linen such as Kylies, sheets, pillowcases, towels will be supplied and cleaned. It is not for personal laundering.
Deliveries and collections are made twice weekly.

Nepean Options uses a Person Centred Planning Approach to identify each service users goals and needs. The case management consultant meets with the service user and together they undertake a comprehensive assessment which will help them to identify that person's goals and the areas of their life that they need assistance with. The case management consultant then negotiates for the services to start and monitors them to make sure that everything is working as expected. The case management consultant provides the service user with a single person they can contact to arrange services they need and resolve any problems or issues that they encounter.

FEE $8.00 for an average sized block (from July 2001)
This is a lawn mowing service providing 18 cuts per year. The service includes mowing, whipper-snippering of edges and sweep-up of paths. Clients are provided with 9 vouchers every six months (to a maximum of 18 per year) to be used as required. Lawn mowing clients may use one or more of their vouchers per year to have their gutters or outside windows cleaned (single storey only). There is a long waiting list for service but referrals will still be accepted and prioritised for crisis cases.
